Got a call form my nephew wanting to know if I could stop by for a few minutes. Upon arrival he takes to his sons layout. Then he ask if I can look at one of his Lion Chief Engines. Upon inspection I could not do anything that I could think of that would fix this problem. Here is the run down of what transpired.
Engine is a Lionel Lion Chief Engine. It is the Brown Engine from Thomas and Friends. I think it is called Henry but not sure.
Symptom is it is DOT. Dead on track.
Put together a short test track and put it on. Test track uses one of the wall warts to plug into a terminal track. Has a red light on it. Light is solid red with NO engines on track. Put any engine except this on track and it works as it is suppose to. Also exchanged wall wart with another to rule wall wart out of equation.
Put this engine on track and red light now FLASHES.
Opened engine to inspect. Found no pinched wires or broken wires or rubbing wires.
Found no indication of burned or heated spots on board or anywhere else.
All Capacitors looked fine.
Re assembled. Same Result.
Any one have any ideas? I believe they were going to take the ride to Just Trains of Delaware today and have them look at it since they are the closest dealer to us in South Jersey. But my own curiosity is really screeching loudly. Just curious if anyone had any ideas.

Most likely the board is bad. Those wall warts have a really quick shut off time, that is what prevents the board damage. If you put it on a regular layout with a 10 or 20 amp breaker and it derails just right then boom, bad board. I've had to replace a few of these. The good news is that it's pretty straight forward to swap one of these out...

Can you post the model number of the engine? It might help in diagnosing the situation.
