Nothing like that just the occasional coupler that comes open and if I don’t catch it on time get a pile up and then the Big Hand has to do the clean up.

I don't know where the photo is, but once upon a time I was running 4 TMCC locomotives (light) on the Panhandle 1 (18'x10') mainline (a folded dogbone). I couldn't switch among them fast enough to keep them apart and had a rear-end collision. Damn NTSB showed up and I had to pee in a cup. George

I usually run three trains using two DCS remotes. One remote controls a train on the upper mainline and one on the lower mainline while the other remote controls the second train on the lower main. The lower main is a big E shaped loop and it's easy to keep the two trains spaced. I've even run three trains on the lower main but it's not as relaxing as running two so I usually don't run three. Normally all is well and there is no problem keeping the trains apart, but one time I was running the Aerotrain on the upper main and wanted to use the passenger announcements at the passenger platform. I thought I had switched control from the passenger train on the lower main to the Aerotrain and hit the passenger announcements button, then the direction button as the train approached the platform. Nothing happened. I could hear the announcements but the train was still running. It took a few seconds to realize I had stopped the E8s and was hearing their passenger announcements and the PAs crashed into the back of the stopped train before I could react. Cars derailed and shorted popping the circuit breaker. The crash occurred behind the buildings in the city. I had to get a chair to stand on to reach the derailed trains, not an easy task, but in the end all was well. MTH trains are pretty rugged and nothing was damaged.
The moral of the story is that when something doesn't make sense, like then the announcement says, "Now arriving on track one, the Chicagoan" rather than, "Now arriving on track one, the Great Lakes Aerotain" rather than wondering why the train isn't stopping and the announcement is all wrong, HIT THE EMERGENCY STOP!!!
P.S. The crash on my layout wan't nearly as spectacular as those in the movies. There was no explosion, no fireball, no O gauge citizens running for their lives, no trains crashing through buildings or any other special effects.

I gave my CAB 2 remote to my little grandson. Before I could blink he had the speed knob up all the way. I could see my brand new SD90MAC heading for a curve and was imagining what would happen next. I took both hands and lifted it off the track, no damage done. I didn't say anything but the little guy went into a corner and hid, he was so upset with himself.

I've got 3 oval loops and at least twice, when I wasn't paying attention, the consist separated at a weak coupler and the engine slammed into the back of the last car. Never any damage, but I do now have a few of the older couplers zip-tied now.

Maybe about a year ago, I almost had a disaster, and if it did happen, probable wiped out my town. Was running 3 trains, 2 on 1 track and switched back and forth between outer and 2nd loop. Also running a short train with switcher on another rail. For some reason, my MTH Pennsy Sharks started up, no sound, just light and creeping into mainline. Luckily the switch sound made me look just as Pennsy had switch to mainline and coming head on was my 2-8-0 Pennsy Freight train. I hit the power button, when I looked the disaster was a so close, a penny would not have fit in between the 2, and a 3rd train coming behind. Never did figure/find out how the Sharks started. Never happened again since. I have several "Panic" buttons as I call them mounted on layout to kill power, just in case and glad I did then even though my handheld has them also.

I had an 25 car intermodal train derail. And about 8 cars fell to floor and what a mess I had.
